The Regional Health and Human Services Agenda is a community-based approach to improve population health and strategically identify areas of greatest need and collaborative opportunity. To successfully implement it, the Foundation uses data to inform our strategies; establish a benchmark for our efforts; and measure progress over time.
Community Memorial Foundation has created a platform that provides continually updated health, economic and quality-of-life population data specific to our 27 communities. These analytics can be used to prioritize needs and track progress against national and locally-defined targets. They inform the Foundation’s grantmaking strategies and can be used by grantees to implement evidence-based programming that meets the specific health needs of our community.
